Give your people somewhere safe to report

Anonymous workplace incident reporting is a workplace channel that lets staff report hazards, misconduct or unsafe behaviour without providing their name or any detail that identifies them.

Without one, hazards don't just disappear. Staff vent to each other, nothing gets logged, and you find out at the exit interview, the compensation claim, or the inspector's visit.

With Foremind you get a clearer picture of what's happening, and a documented record of how you responded.

How it works

Report

Any staff member with a Foremind account opens the platform on their phone or desktop, describes what happened, and selects the hazard type.

Notify

The report appears immediately in your admin dashboard. No inbox, no email chain, no paper form.

Act

You link the report to a risk, assign a rating, record the control you put in place, and set a review date.

Admins can also log risks directly, so both staff reports and management observations feed the same record.

I've noticed that the team's morale has been at an all-time high this year, and I think a lot of that has come from having a service like Foremind in place.

Foremind’s work is inspirational. We are all interested in improving positive mindsets and wellbeing.. and workers don’t always have the opportunity to have that wellbeing check in. There’s not that culture of putting your hand up and saying you’re not okay.

Ive been able to visibly notice a change in energy and productivity in the office since engaging Foremind, the onboarding and initial training were so fantastic we decided to do it quarterly.

“Having the ability to provide support for our staff has been really beneficial. The feedback we always hear is how easy it is to book a session. They can go in, look at who is available and book it in quickly. It's been really great.”— George, Vacenti

How anonymous is anonymous?

Completely. Foremind does not collect a name, an email address, or any identifying detail.

There is no hidden field for an administrator to unlock, because the information was never captured. That is a step beyond confidential.

Confidential means the organisation knows who you are and promises not to tell. Anonymous means there is nothing to tell.

Your admin sees what happened, when, and which hazard type it relates to. That is the whole record, and it is enough to act on.

Compliance

How it supports your psychosocial duties

Under the Work Health and Safety Act 2011 and its state equivalents, a PCBU must eliminate or minimise psychosocial risks so far as is reasonably practicable, and must consult workers in doing so.

Anonymous reporting is one of the few consultation mechanisms that reaches the hazards workers will not raise in a team meeting.

Regulators don't just ask what you did. They ask what you knew, and when.

Identify and assess

Reports come in tagged by hazard type and link straight to your risk register.

Control and review

You record the control, attach evidence, and set a review date the platform reminds you about.

Documented as you go

The four-step Code of Practice cycle, evidenced at each step, not reconstructed for an audit.

Report on every hazard type

Staff can report any psychosocial hazard, from bullying and aggression through to workload, low job control and unclear roles, plus physical safety hazards, near misses and injuries.

Reporters tag the hazard type when they submit, so three separate reports about workload stop being three grumbles and start being a pattern you can evidence.

Keeping both kinds in one place matters, because they rarely arrive separately: fatigue shows up as a near miss, understaffing shows up as an injury.

See common psychosocial hazard examples for the full set.

Reporting combined with support

Most tools capture the incident and stop there.

Foremind's employee assistance program means the person carrying it can book professional counselling through the same platform, in their own time, without telling anyone.

No, but the underlying duty is. Under the Work Health and Safety Act 2011 and state equivalents, a PCBU must minimise psychosocial risks so far as is reasonably practicable and consult workers. An anonymous channel is a defensible way to show you gave people a real way to raise concerns.

Staff don't report psychosocial issues because they fear being identified, so problems escalate silently until they become a claim or incident. Anonymous reporting gives you early visibility to act before that happens.

Every report, action taken, and control implemented is documented and exportable. Your audit trail is built automatically as you work.

No. Anonymous reporting sits outside your formal grievance process. It's designed to surface issues early so you can act before they escalate to a claim or incident.
